• Physical exams are required for all students entering Kindergarten. They are also required for grades 1, 3, 5, 7, 9, 11. It is extremely useful to hand in an updated Physical Exam Form every time your child has an annual Physical exam with their health care provider in order to keep their school health records up to date.

• As your child receives vaccines it is important to hand in proof of the immunization to keep their school health records up to date.

• Keep the school nurse updated with any pertinent health history, surgery, allergies, medications or anything you feel would be useful. 

• Children are permitted to take medication during school hours if certain requirements are met. This requirement applies to all prescription and even over-the-counter medications (including cough drops). A doctors order is required or your doctor may complete a Permission to administer medication at school form. Parents are required to transport all medications to school. Prescription medication must be in original pharmacy labeled container. Discuss this with your child's health care provider and the school nurse. 

• In the event a student is unable to participate in gym activities due to illness, injury etc. a parent/guardian may request this in writing to the school nurse for one day. Anything beyond one day requires a written note from your health care provider. Anytime a student is unable to participate in gym activities they are not allowed to participate in recess.

• In the event your child is hospitalized, had surgery or sustained an injury requiring medical care please have your child’s health care provider complete a Return to School form or at least paperwork that indicates when they are able to return to school and when they are able to participate in gym activities. Please contact school nurse for more information should an event occur.

• Temperature greater than 100 degrees. Student needs to be fever free for 24 hours and not taking fever reducing medication prior to returning to school

• Vomiting or persistent diarrhea. Student needs to be symptom free for 24 hours prior to returning to school

• Diagnosed with strep infection. Student needs to be on antibiotic for a minimum of 24 hours and fever free before returning to school

• Frequent, congested (wet) or croupy cough, chest congestion or any signs of respiratory distress

• Pink eye (conjunctivitis) - red, itchy eyes with crusting or drainage. Student can return to school after being cleared by a health care provider

• Live head lice - student can return to school after being treated according to health care provider's instructions

• Certain skin conditions – student can return to school after being cleared by a health care provider

The Health Office supports students' academic success by promoting health in the school setting. One way we provide care for your child is by performing the health screenings as mandated by the State of New York. Screenings will not be done if adequate screening results have been provided on a Health Examination Form from a licensed medical provider. 

During this school year, the following screenings will be required or completed at school:

Vision

• Distance and near acuity for all newly entering students and students in Pre-K or Kindergarten, Grades 1, 3, 5, 7, and 11
• Color perception screening for all newly entering students

Hearing

• Hearing screening for all newly entering students and students in Pre-K or Kindergarten, Grades 1, 3, 5, 7, and 11

Scoliosis

• Scoliosis (spinal curvature) screening for all girls in grades 5 and 7, and boys in grade 9
